Main duties of the job
An exciting opportunity has arisen within East Hertfordshire Adult Community Mental Health Services to be part of the Physical Health Team providing services to people with Severe Mental Illness. Cygnet House is looking for an individual who is enthusiastic, passionate, motivated and one who is willing to make a difference to their community, supporting patients with mental health difficulties. You will work as part of a multi-disciplinary Team, working in partnership with a range of stakeholders inside and outside of the Trust under the clinical supervision and oversight of the Clinical Nurse Specialist. The post holder will be undertaking physical health checks, and well-being assessments, within the larger context of the new transformation of mental health support in East Hertfordshire.
The post holder with partake in the Clozapine clinic taking weekly bloods and monitor levels.
The post holders will assist in enabling patients to access the support they need to recover from their mental health and physical health problems, promoting and maintaining recovery. They will also support the locality in ensuring the Severe Mental Illness (SMI) registers are updated.

Detailed job description and main responsibilities
The post holder will contribute to assessment of both physical and mental health needs. Focusing on the physical health and wellbeing of our patients with mental health needs and have an understanding of the relationship between physical and mental wellbeing.
The post holder will carry out venepuncture for requested blood tests and partake in the physiological assessment of our patients who have been identified as having severe mental illness. They will take and record physical health checks including blood pressure, pulse, weight and BMI.
To provide information on health promotion including healthy eating, exercise and smoking cessation, presented in a way that is meaningful for the patient.
To work with patients and relevant staff to develop physical health care plans and complete the electronic physical health assessment form on care notes, ensuring the information is shared with patients and GP’s
Work closely with the physical health team leader to set up clinics across the locality and communicate progress effectively with local teams

Qualification and Training
Essential criteria
- NVQ Level 3 Qualification or mental health cert level 3 / or equivalent or agreement to undertake training
- NVQ / MH Certificate Level 2
- Venepuncture

- Car driver (Unless you have a disability as defined by the equality act 2012 which prevents you from driving
Communication Skills

Experience
Essential criteria
- Three years’ experience of providing mental health or health and social care services / evidence of transferable skills
- Good verbal and written communication skills
