Main duties of the job

At the direction of the Registered Nurse, participate fully in the delivery of planned nursing care, using a wide range of clinical skills.Participate in the transfer and discharge of patients.Assist in all aspects of personnel hygiene and toileting needs of patients Assist with the service of meals and the feeding of patients as needed.Assist with the safe moving, handling and mobilisation of patients.Involve patients and carers/relatives in delivery of patient care.Provide reassurance and support to patients, carers/family.Preserve clients privacy and dignity at all times.Recognise and respond appropriately to urgent and emergency situations.Provide information to enable the effectiveness of care to be evaluated and records maintained.Assisting with personal carer of patients in relation to toileting and moving and handling.Maintain a professional working relationship will all members of the multi-disciplinary team.The postholder will maintain the confidentiality of Trust and patient information unless authorised for specific purposes. Report any concerns about the use of such information to the Sister/Nurse Recognise and report defects in buildings and equipment to Centre Manager/Nurse in charge or other relevant personnel.

Clinical

  • Involve patients and carers/relatives in delivery of patient care.
  • Provide reassurance and support to patients, carers/family.
  • Preserve clients privacy and dignity at all times.
  • Recognise and respond appropriately to urgent and emergency situations.
  • Provide information to enable the effectiveness of care to be evaluated and records maintained
  • Participate in the maintenance of accurate records of care.
  • Work collaboratively with others in the ward/ Specialist Palliative Care Team to ensure that patients needs are met.
  • Ensure the effective and efficient use of resources.

Person Specification

Qualifications

Essential

  • NVQ Level 2 in a Health related subject
  • Level 1 Functional Skills/Keys Skills in Numeracy and English or to achieve within 12 months of commencement in post
  • For existing Mid Yorkshire staff, evidence of equivalent experience, including SIPP2 and Core Competencies, will be acceptable for the above.

Desirable

  • NVQ Level 3 in Health related subject

Experience

Essential

  • Excellent verbal / non-verbal communication skills.
  • An ability to demonstrate an understanding of role and responsibilities.
  • Previous experience in the health care setting

Desirable

  • Previous patient care experience in palliative care
