If so, we have an exciting opportunity for you to join our ever-expanding Dementia and Delirium Services at Liverpool University Hospital Foundation Trust (LUHFT) as a band 4 Dementia nurse Practitioner

You will be joining a service that is expanding and focussed on an ambitious three-year plan to develop equitable dementia care across Aintree, Broadgreen and The Royal Liverpool Hospitals. You will work with a motivated and supportive team where your contribution will be highly valued. You will be able to play a significant role in helping to continue the development of the service model and in ensuring people with dementia receive high quality care.

The ideal candidate will have the right skills, knowledge, and clinical experience of working with people who have dementia and delirium. You will be a highly motivated practitioner who has knowledge, expertise, and experience of working within dementia and Delirium within clinical settings and can support patients by delivering fundamentals of care including diversion, distraction and reminiscence therapies.

Main duties of the job

Deliver clinical and therapeutic care, within the clinical setting within set protocols and within scope of Assistant Practitioner remit.

Organise and prioritise time effectively.

Perform patient assessment (under supervision and after appropriate delegation from the registered practitioner),and deliver high standards of care in line with the Plan of care

Implement and evaluate individual care/treatment plans. Ensure care is documented accurately and in a timely manner as per Trust Policy.

Qualifications

Essential

  • Foundation degree in assisting professional practice/equivalent

Experience

Essential

  • Previous acute care experience
  • Ability to provide excellent patient care

Knowledge

Essential

  • Ability to analyze patient progress in response to care given
  • Understands the role of audit in assuring standards of care and can undertake audits in practice.

Skills

Essential

  • Excellent communication skills
  • Ability to develop effective interpersonal relationships with colleagues in the Healthcare setting.
  • Basic Computer Skills: I/E can use PAS systems, email and can access policies on the Trust intranet
